What backup power does a Pakistani pharmacy or clinic need?

A Pakistani pharmacy/clinic needs three protection tiers: an online double-conversion UPS (3-15 kVA) for vaccine fridge + medical equipment continuity through load-shedding; an ATS-controlled diesel generator (8-25 kVA) for full-site backup; and 30 mA RCBOs on every patient-contact circuit per IEC 60364-7-710 "Medical Locations" requirements. Total backup investment for a typical 5 kW pharmacy: PKR 150K–350K. For a 15 kW clinic: PKR 400K–800K. Investment justification: a single vaccine-fridge failure during 4-hour outage can spoil PKR 100K-500K of stock; one outage event pays for the UPS.

UPS-critical total: ~425 W — size online UPS at 1 kVA (2-3× headroom for inrush) with 30-min battery autonomy. For a small clinic add ultrasound (250 W), nebuliser (300 W), suction pump (300 W) and oxygen concentrator (350 W) → ~1.5 kVA UPS.

Plus a small standby generator (3 kVA petrol or 5 kVA diesel = PKR 70-130K) brings total backup investment to PKR 125-185K. For a multi-room clinic add 15 kVA generator + manual changeover (PKR 220K) and 3 kVA UPS for medical equipment.

Why 30mA RCBO is mandatory in medical locations

IEC 60364-7-710 "Medical Locations" requires:

  • Group 0 (waiting room, office): standard 30 mA RCD/RCBO on sockets — same as residential.
  • Group 1 (consulting room, X-ray, dialysis): 30 mA RCD on socket circuits + redundant earthing + equipotential bonding.
  • Group 2 (operating theatre, ICU, cardiology): Medical IT system (isolated, ungrounded) with insulation-monitoring — standard RCBOs replaced with line-isolation monitors.

Most Pakistani pharmacies and small clinics fall under Group 0 / Group 1, where 30 mA RCBO on every socket is sufficient. Hospital ORs and ICUs need the Medical IT system — outside scope of this guide.

Common Pakistani pharmacy/clinic installer mistakes

  1. Vaccine fridge on a regular line-interactive UPS — switchover transient kills compressor. Use online double-conversion only.
  2. Skipping voltage protector at main — broken-neutral 415V destroys all electronic equipment simultaneously.
  3. 30 mA RCBO on the geyser — nuisance trips. Use 100 mA on geyser circuits.
  4. Sharing UPS output with AC — AC compressor inrush trips UPS bypass. Separate AC to generator-only side.
  5. No SPD on UPS output — lightning surge propagates through UPS to medical equipment. Add Type 2 SPD downstream.
  6. Cold-chain failure logging missing — if vaccine spoils, you need temperature log to claim insurance. Use a WiFi temperature data-logger.
